Permission Form

In order for a skater to participate in a U.S. Figure Skating sanctioned event (including test sessions) at a club other than his/her home club, this form must be completed by an officer of the skater’s home club and sent to the club hosting the event.

Please include this form along with receipts to receive reimbursement for expenses incurred on behalf of the Club.

Volunteer Log

It is the responsibility of every Birmingham Figure Skating Club member to log your volunteer hours and volunteer at least 10 hours each year. You will need to turn in this log by the end of the calendar year with your skater incentive application in July, and to be eligible to receive skater incentive each family must volunteer 15 or more hours.
